Knothole Island begins by going to the docks in Bowerstone. There you’ll find a guy with his own submarine – a rather sophisticated device for the vaguely medieval Albion – who is willing to take you to Knothole Island, which lies somewhere off the coast. When you get there, you find a small village stuck in permanent winter, led by a chieftain desperate for your help (but perpetually unwilling to give you credit for your help) in fixing the situation.
